I have the Sniper perk - its supposed to add accuracy to head shots. So does the percentage meter in VATS take this into account? Or should I assume that the perk comes into play on top of whatever percentage accuracy it is showing me for their head?

I believe the percentage shown includes the perk.

One thing to remember is the bonus isn't additive, it's multiplicative, so you won't see a straight +25% difference.
